Five Wishes Support

Offered as one of our consulting services, The Five Wishes, is often referred to as “a living will with heart and soul.”  They are brought to us by the nonprofit organization Aging with Dignity. This document can help you to find peace of mind about the way you would want to be cared for, should you be unable to speak for yourself due to injury or illness. Five Wishes meets the legal requirements in forty-two states, including Arizona. It is both a durable power of attorney for health care and a living will. Coaching Sessions can be designed for individuals, families, or groups of friends with the common goal of completing this important document. Five Wishes reflects details about the following areas:

  • The person to make health care decisions for you if you cannot speak for yourself
  • The kind of medical treatment you want or don’t want
  • How comfortable you want to be
  • How you want people to treat you
  • What you want your loved ones to know
